Recently, it has been demonstrated that the QT/RR relationship pattern varies significantly between healthy individuals. This study compared the intra- and inter-individual variations of the QT/RR relationship.
Methods: 24-hour 12-lead digital ECGs (SEER MC, GE Marquette, 10-second ECG recorded every 30 seconds) were obtained at baseline, after 24 hours, 1 week and 1 month in 75 healthy subjects (42 female, 33 male, age 27.9±9.6 vs 26.8±7.5 years, p=NS). QT interval was measured automatically in each ECG by 6 different algorithms and the mean of the 6 measurements was analysed. In each recording of each individual, the QT/RR relationship was assessed by 10 different regression models including the linear (QT=
+ßxRR) and parabolic (QT=ßxRR
) models. The standard deviations (SD) of the regression parameters
and ß of consecutive recordings of each individual were compared with the SD of the individual means. The intra-subject stability and inter-individual variability were further tested by ANOVA.
Results: With all models, the intra-individual SDs of the regression parameters were highly significantly smaller than SD of individual means (p<10-5-10-9). The intra-subject stability was further confirmed by ANOVA (p<10-19-10-30).
Conclusions: The QT/RR relationship exhibits substantial inter-subject variability as well as a high intra-subject stability. This has practical implications for a precise estimation of the heart rate-corrected QT interval in which optimised subject-specific rate-correction formulae should be used.
